Passport is a reimbursement program, which means you go out and purchase the services and supports you want, and then send your receipts in to PassportONE on the claim forms we will provide you.
There are guidelines that explain what you can purchase and be reimbursed for. You can get a copy of the Ministry of Children, Community and Social Services at this link: Passport Program Guidelines for Adults with a Developmental Disability and their Caregivers (April 2023).
When you are approved for funding, our office will be contacting you to confirm your address and other information. We will then mail you your onboarding package. The onboarding information provides you a step-by-step overview of the information you need and the forms you need to sign in order to activate your Passport funding.
You can request an onboarding information session with one of our Passport staff members and they will walk through the information with you and answer any questions you have.
You will be assigned a Passport Coordinator who will be your point of access if you have questions about your funding and reimbursements.
